I'm going to have to dig out my copy. If I remember correctly, it was a separate magazine published solely to promote Bag of Bones. I thought I had a picture, but I can't find it. When I dig out my copy, I'll scan it and post it.

No, LSOE had additional artwork created for it that was not in the earlier printings of The Gunslinger.

The 3rd edition has a new cover, but is otherwise the same as the 1st and 2nd editions.
