Thanks Michaël. I was told by the original recipient of the proof there were only a dozen made. It was someone I trust with the numbers. I suppose a limited edition of a previously released book does not require many proofs, if any.

Maybe the copies sent to reviewers had ‘NOT FOR SALE’ stamped on them. Possibly spare copies were not stamped?
