I hardly ever offer an opinion here because I don't study signatures, but that's the worst godawful thing I've seen in ages.

It looks like a typical SK missive.
He was also under the weather, which might contribute to the condition of his signature.
It's signed in an unusual place -- authors generally sign on the title page.
Possibly Kirby McCauley's sister -- he was representing King at the time, and she was also part of his agency.

The writing neither looks nor sounds like King.
It's a good observation about the angle -- my autographs are never straight unless I'm signing a limitation page where there's a line to guide me.
Ha! You'd think that with an example of the signature RIGHT THERE ON THE FREAKING COVER that whoever did that would have done better.
I think the term pertains to a revised edition (the revision could simply be a new cover) from a different publisher or printing house than the original. Otherwise it would just be a revised edition.
Seeing as how Steve sued to have his name taken off this movie, I can't imagine him wanting to put it back on again!
