Not to split hairs but the Centipede version you are referring to is the unsigned gift edition (600 total) and does not include the Jerry Uelsmann photographs except for one opposite the title page.

When referring to the book title it is 'Salem's Lot. When referring to the town it is 'salem's Lot unless at the beginning of a sentence.
If you are a fan of the movie it's worth getting. Many interesting interviews and insights into how it was made and the impact it had on TV in the late 70s.

That's a tough one, mainly because the prints almost never show up. The Collector made a total of 200 of the cases, so those are fairly scarce also. I'd say somewhere in the $500-750 range.

The unsigned 600 copy edition went for $125 when published and probably sells somewhere in the $150 range. The Betts edition likely has better binding material and has the limited edition Mohawk...
