All reputation is anonymous, unless disclosed in the Comments by the sender.
Admin staff with access to the Admin Control Panel can see who left comments.
Without some add on 3rd party software I can't make the reputation reveal the sender.

I believe the options are: "I approve" or "I disapprove". Very slight difference.

Looking into the matter, but apparently you are correct.
I believe a member can give negative rep, but it cannot be anonymous.
You can't send negative rep without disclosing yourself, however you can click positive, then leave a chiding remark.
Under "Notifications" you should click "View Reputation" and get a screen that looks like this:
