King started with outlines, but over the course of 30 years things get misplaced. Now, there is little excuse for mistakes within a single book. That is, indeed, the editor's job.

I vote revised, but I've never read the original. From what I've heard, it sounds like the changes were an improvement.

Marvel often numbers one-shots. I would guess for consistency. One-shots can be longer, but not always. The Giant-Size label indicates length.
