I think this conversation is about what constitutes an artist rather than who should sign. It seems everyone agrees only the artists involved should sign.

I like the idea of a signature page that includes everyone involved. Sort of like when the illustrator signs too, but this time you'd have the engraver, woodworker, binder, etc. Would be cool to see...
